Client Services Agreement
This agreement is between you (the Client) and Michelle Weeks (the Coach). By sending a payment to the Coach, clicking “I have read and agree to the terms above” or by using the Services you agree to be bound by this Agreement.
1:1 Life Coaching Series (the Services) includes:
- 6 x 1:1 coaching sessions with the coach held via Zoom that are 60 minutes each in duration. An audio recording of each coaching session will be provided upon request. All coaching calls are to be scheduled and completed within a 90 day period from the purchase date of the program.
WhatsApp Support between your coaching sessions.
(Mon-Friday. Reply Time is 24 hrs.)
180 days of member portal access to Kajabi to track progress, record notes, receive coaching call recordings and set action steps.
The total payment for the service is $2000 AUD.
Your payment is non-refundable and non-transferable. If a refund is requested, the Coach reserves the right, at their sole discretion, to grant or deny any refund request. You understand that your inability to attend your coaching session/s does not qualify for a full or partial refund.
You agree to be coachable, to communicate honestly, to be open to feedback and to create the time and energy to participate fully in the program. You agree that it is your responsibility to be available for each coaching session at the scheduled time. In the event that you are late for a coaching session, the Coach reserves the right not to reschedule or extend the coaching session. The Coach agrees that it is their responsibility to be available for each coaching session at the scheduled time. In the event that the Coach is less than 15 minutes late for a coaching session, the coaching session will be extended. In the event that the Coach is more than 15 minutes late for a coaching session, you will have the option to reschedule your coaching session. You agree that it is your responsibility to notify the Coach at least 12 hours in advance via email if you are unable to attend a scheduled coaching session. The Coach will attempt in good faith to reschedule the meeting. The Coach reserves the right not to reschedule the coaching session if adequate notice is not given. In the event that the Coach is unable to attend a scheduled coaching session, the Coach will notify you at least 12 hours in advance via email and the coaching session will be rescheduled. You agree that you are solely responsible for your own physical, mental and emotional wellbeing, decisions, choices, actions and results arising out of or resulting from the coaching relationship. You agree that the Coach is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of the Services provided by the Coach. You understand that coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure to treat any mental disorder or medical disease.
This coaching relationship, as well as any information (written, verbal or recorded) that the Client shares with the Coach as part of this relationship, is bound by the principles of confidentiality. The Coach agrees not to disclose any information pertaining to the Client without the Client’s written consent. The audio recordings of your coaching sessions are for your personal use only. You agree not to share, sell, distribute or publish these with any party, by any means, including after the Services have been completed, suspended or terminated. Emails from the Coach relating to the Services are for your personal use only. You agree not to share, sell, distribute or publish the emails with any party by any means, including after the Services have been completed, suspended or terminated. You acknowledge your continuing obligating to raise any confidentiality questions or concerns with the Coach in a timely manner.
